生长抑素14肽对胰十二指肠切除术后胰瘘的预防作用

目的观察生长抑素14肽在胰十二指肠切除术后抑制胰液的分泌、预防和治疗胰瘘的作用。方法回顾性分析本院2002年1月至2010年11月收治的进行胰十二指肠切除术患者共42例,全部病例术后常规肠外营养及抗生素治疗,其中24例术后预防性应用生长抑素14肽作为试验组,与18例术后未用者进行对照,比较2组术后并发症和胰瘘的发生率、胰液分泌量及胰周淀粉酶含量。结果治疗组的术后并发症和胰瘘的发生率、胰液分泌量以及胰周淀粉酶含量明显低于对照组,差异有统计学意义。结论用生长抑素14肽能降低胰十二指肠切除术后并发症的发生率。

Preventive effect of somatostatin on pancreatic fistula in pancreticoduedenectomy

Objective To explore the effect of somatostatin on inhibiting pancreatic secretion as well as preventing and curing pancreatic fistula in patients underwent pancreatodudenectomy.Methods The data of 42 pancreticoduedenectomy patients were collected retrospectively and analyzed.Somatostatin was prophylactic used in 24 cases as treatment group.The other 18 patients did not receive any somatostatin after operations as control.Conventional parenteral nutrition and antibiotics were used in all patients.The incidence of pancreatic fistula,the quantity of pancreatic juice and peripancreatic amylase were analyzed.Results The postoperative complications and the incidence of pancreatic fistula,the quantity of pancreatic juice and peripancreatic amylase in treatment group were significant lower than those of the control group(P<0.05).Conclusion The prophylactic use of somatostatin are necessary and can reduce the incidence of postoperative complications of pancreatodudenectomy.
