无注气内镜下行甲状腺手术15例报告

目的探讨无注气内镜下甲状腺手术的可行性.方法对15例甲状腺肿物患者取胸骨柄上2cm水平切口入路,局部麻醉加内关穴位强化麻醉,在内镜照明下联合超声刀完成甲状腺手术.其中行甲状腺次全切除6例,腺叶加峡部切除9例.结果手术在1～2 h内完成,术中出血量10～30 ml,无并发症发生,术后美观效果好.结论无注气内镜下行甲状腺手术,利于术中观察识别喉返神经和甲状旁腺,术后美观,是甲状腺微创外科手术的一种新方法.

Curative effect discussion of the endoscope thyroid gland operation with no gas injection

Objective To probe into the feasibility the endoscope thyroid gland operation with no gas injection. Method Among all 15 thyroid patients, by breastbone handle 2era horizontal notch entry way local anaesthesia with close the point insideis were anaesthetize to strengthen,thyroid gland operation was done with unite ultrasonic knife under inclusive mirror loght inside, thyroid gland the majority etcises operation was 6 cases, gland leaf and gorge department excise was 9 cases among them. Result All operation were finished in 1 -2 hours, bleeding in operation was about 10ml to 30ml, there was no complication to happen, notch appearance was beautiful and effectual after operation. Conclusion Endoscope thyroid gland operation with no gas injection is beneficial to observe discerning the laryngeal nerve and parathyroid, using ultrasonic knife can make operation patient bleed less, it is the new method of the thyroid gland small wound surgery.
