应用Sonoliver定量分析及受试者工作特征曲线评价超声造影对眼部良恶性肿块鉴别诊断的价值

摘    要:目的:探讨超声造影Sonoliver定量分析及受试者工作特征(ROC)曲线在眼部良恶性肿块鉴别诊断中的应用价值。方法:对37例眼部肿块患者术前行超声造影检查(良性26例,恶性11例),应用Sonoliver定量分析,获取时间-强度曲线(TIC)参数及动态血管模型(DVP),以病理结果为金标准,分析超声造影在鉴别诊断眼部良恶性肿块的价值。结果:①TIC及DVP全面显示了肿块的微血管灌注模式及血流分布,对良恶性肿块具有鉴别诊断意义。②TIC参数中上升时间(RT)、造影灌注指数(PI)、上升支斜率(α_1)、半降斜率的绝对值(α_2)、mTT两组间差异有统计学意义(P<0.05),参数中达峰时间(TTP)、造影剂到达时间(AT)及峰值强度(IMAX)两组间无明显差异(P>0.05)。③通过ROC曲线分析发现α_1≥67.5 dB/t,α_2≥22.2 dB/t、PI≥528.1a.u,RT≤8.6 s,mTT≤29.8s为诊断肿瘤恶性的最佳截点(cut-off)。结论:应用Sonoliver定量分析获得的时间-强度曲线及动态血管模型可对眼部肿块进行定量分析获取参数,对眼部良恶性肿块的鉴别具有一定的诊断价值。

Abstract:Objective:To investigate the value of Sonoliver quantitative analysis and receiver operating characteristic(ROC) curve in contrast-enhanced ultrasound(CEUS) diagnosis of ocular masses. Methods:A total of 37 cases ocular masses(26 cases in benign group and 11 cases in malignant group) were examined with CEUS before surgery,and the time-intensity curve(TIC) and dynamic vascular pattern(DVP) using Sonoliver Software were analyzed.The results of CEUS were compared with the pathological examination,and the differential diagnostic value of CEUS was analyzed.Results:①TIC and DVP were helpful for the differential diagnosis of the benign or benign mass.②The rise time(RT),perfusion i ndex(PI),α_1,α_2 > mean transit time(mTT) of TIC were significant different between the benign and malignant groups(P<0.05).While time to peak(TTP),contrast agent to arrive time(AT),and maximum intensity(IMAX) of the TIC had no significant difference(P>0.05).③By means of ROC,we found the cut-off the malignant mass isα_1≥67.5 dB/t,α_2≥22.2 dB/t,PI≥528.1 a.u,RT≤8.6 s,mTT≤29.8 s.Conclusion: The obtained TIC and DVP using Sonoliver software can provide quantitative data for differentiation diagnosis of ocular mass.
