鲍曼不动杆菌临床感染分布及耐药性分析

摘    要:目的了解鲍曼不动杆菌临床感染分布和耐药现状。方法应用纸片琼脂扩散试验(K-B法)对322株鲍曼不动杆菌进行细菌培养鉴定及药敏分析,用WHONET5.4软件分析试验结果。结果2008年1月~2008年12月间共分离鲍曼不动杆菌322株,312株分离自住院患者,10株分离自门诊患者。痰标本分离率最高达85.09%,主要引起神经外科和重症监护病房患者的下呼吸道感染。该菌对米诺环素敏感率最高达68.89%,对头孢哌酮/舒巴坦、美罗培南、左旋氧氟沙星、亚胺培南和头孢吡肟的敏感率分别为46.67%、44.44%、43.42%、42.22%和40.12%,对哌拉西林/他唑巴坦、复方新诺明、哌拉西林、庆大霉素的敏感率均在30%以下。结论鲍曼不动杆菌感染以神经外科和重症监护病房最常见,该菌对米诺环素敏感率最高。

Abstract:Objective To determine the distribution and resistance of clinical isolates of Acinetobacter baumannii.Methods The bacterial susceptibilities to 14 kinds of antimicrobial agents were determined by Minimum inhibitory concentrations(MICs).The results were analyzed with WHONET 5.4 software.Results A total of 322 strains were isolated during the period of January 2008 to December 2008,312 strains of them isolated from inpatients,and 10 strains of them isolated from outpatients.The isolation rate of Acinetobacter baumannii was 85.09% in sputum specimens,and majority of the isolates caused lower respiratory tract infection in older inpatients and serious wounded ones.68.89% of them were susceptible to minocycline and the susceptible rate to cefoperazone-sulbactam,meropenem,levofloxacin,imipenem,cefepime is 46.67%,44.44%,43.42%,42.22%,40.12%.Under 30% of them were susceptible to piperacillin-tazobactam,piperacillin,trimethoprim-sulfamethoxazole,gentamicin.Conclusions The infection of Acinetobacter baumannii is most serious in neurosurgery and ICU.It is most susceptible to minocycline.
