进展性缺血性脑卒中的相关危险因素临床分析

目的探讨进展性缺血性脑卒中（PIS）的危险因素,为PIS的预防和治疗提供参考。方法对202例脑梗死患者的临床资料进行回顾性分析,对PIS发生的相关危险因素进行Logistic回归分析,筛选PIS发生的相关危险因素。结果单因素分析结果显示,糖尿病史、NIHSSS评分、全血黏度低切、全血黏度中切、全血黏度高切、纤维蛋白原、葡萄糖、高密度脂蛋白与PIS发生有关,而性别、年龄、吸烟史、高血压史、冠心病史、入院时体温、入院时收缩压、入院时舒张压、胆固醇、甘油三脂、低密度脂蛋白等因素与PIS发生无明显相关性（P〉0.05）;多因素Logistic回归法分析表明,糖尿病史、全血黏度、纤维蛋白原、高脂血症、NIHSS评分等5个因素是PIS发生的危险因素。结论糖尿病史、全血黏度、纤维蛋白原、高脂血症、NIHSS评分等5个因素是PIS发生的危险因素,对这些危险因素加以重点评估和合理控制,可以控制PIS的发生。

Clinical study on risk factors of progressive ischemic stroke

Objective To investigate the risk factors of incidence of progressive ischemic stroke(PIS) and provide reference for prevention and treatment of PIS.Methods 202 cases of patients with cerebral infarction were studied,analyzing and screening the risk factors of PIS by Logistic.Results The single-factor analysis showed that the diabetes,NIHSSS score,hemorheoloy(low,mid and high),fibrinogen,glucose and high-density lipoprotein had certain degree of contact with incidence of PIS,while the sex,age,smoking history,history of hypertension,coronary heart disease history,admission temperature,admission systolic blood pressure,admission diastolic blood pressure,cholesterol,triglycerides and low-density lipoprotein was no significant correlation with incidence of PIS(P>0.05).Multivariate Logistic regression analysis showed that the diabetes,whole blood viscosity,fibrinogen,hyperlipidemia and NIHSSS score were risk factors of incidence of PIS.Conclusion The diabetes,whole blood viscosity,fibrinogen,hyperlipidemia and NIHSSS score were risk factors of incidence of PIS,assessment and control these risk factors can reduce the incidence of PIS.
