彩色多普勒超声在诊断不全流产中的应用及血流频谱异常的分析

摘    要:目的 提高彩色多普勒超声在不全流产中的应用技巧及对其血流频谱的认知度,增加不全流产的诊断准确性.方法 对彩色多普勒超诊断的不全流产的病例进行总结、分析,并结合临床诊断及病理诊断进行对照.结果 120例临床及病理诊断不全流产者,112例在宫腔高回声光团内见血信信号,探及的血流频谱因不全流产后就诊的时期不同,血流频谱的峰值流速及阻力指数有明显变化,但以低阻、低流速的滋养动脉频谱为主,有5例因过度肥胖、子宫后倾后屈未检出血流信号,3例因光团过小未检出血流信号.结论 经腹彩色多普勒诊断不全流产,只要掌握好彩色多普勒血流显示方法,检出率可达93.3%,当光团内检出滋养动脉血流时,其准确性达100%;血流频谱只是以低速、低阻的滋养血流频谱为主,因不全流产时间的延长,光团内可检出血流速增高,和(或)血流阻力接近正常的变异型的滋养血流频谱.

Abstract:Objective To elevate the application technique and cognition of blood frequency spectrum for ultrasound with color Doppler imaging diagnosing incomplete abortion,and increase its accuratissine. Methods Ultrasound with color Doppler imaging summary and analysis to incomplete abortion cases,cornbine with clinic and pathological diagnosis to contrast. Results One hundred and twenty cases clinic and pathologically approved to incomplete abortion: one hundred and twelve cases had blood signals in high echo mass in uterus,monitored with blood frequency spectrum was different with the time and had drast changes by frequency spectrum peak value and resistance index(RT),majority of low resistance,low flow rate trophoblastic blood flow,eight cases didn't monitor any blood signals,five cases for too obesity and uterine retroversion and retroflexion,three cases for justo minor mass in uterus.Conclusions tran-sit-abdominal ultrasound with color Doppler imaging can diagnose incomplete abortion,if only one master color blood display technique,detection rate was 93.3%,accuratissime rate amounted to 100%,Blood frequency spectrum majors of low flow rate.low resistance trophoblastic blood flow,to monitor the increase of flow rate,and(or)blood resistance a near normal blood frequency spectrum for time.
