Impact of Estimated Pulse Wave Velocity on Risk of Cardiovascular Event and All-cause Death

Abstract:Objectives:To evaluate the impact of estimated pulse wave velocity(ePWV)on risk of cardiovascular event and allcause death.Methods:A total of 101510 employees,who participated in the health examination from 2006 to 2007 were included in this study.Of those,43235 participants with complete baseline data and brachial-ankle pulse wave velocity(baPWV)measurement were selected as the fitted population.Multivariate linear regression models were adopted to fit the ePWV formula,with baPWV as the dependent variable and quadratic age and mean arterial pressure as the independent variables.In addition,98348 employees with complete baseline data were selected as the predicted population.Multivariate Cox regression models were used to evaluate the association between ePWV and risk of cardiovascular event and all-cause death.Results:Participants were followed-up for a mean of 10.3 years.Multivariate Cox regression analysis showed that ePWV was significantly associated with cardiovascular event and all-cause death.After adjusting for the score of prediction for atherosclerotic cardiovascular disease risk in China,the risk of cardiovascular event and all-cause death increased by 32%(HR=1.32,95%CI:1.29-1.35)and 38%(HR=1.38,95%CI:1.36-1.40)respectively for each 1 m/s increase in ePWV.Participants with ePWV≥14 m/s was associated with a higher risk of cardiovascular event(HR=2.69,95%CI:2.52-2.87)and all-cause death(HR=2.62,95%CI:2.47-2.77)as compared to participants with ePWV<14 m/s.After adjusting for traditional cardiovascular risk factors,the risk of cardiovascular event and all-cause death increased by 38%(HR=1.38,95%CI:1.36-1.40)and 53%(HR=1.53,95%CI:1.52-1.55)respectively for each 1 m/s increase in ePWV.Participants with ePWV≥14 m/s faced a higher risk of cardiovascular event(HR=3.39,95%CI:3.19-3.61)and all-cause death(HR=4.21,95%CI:3.98-4.45)as compared participants with ePWV<14 m/s.Conclusions:Elevated ePWV is an independent predictor for cardiovascular event and all-cause death in this cohort.
