急性呼吸窘迫综合征患者机械通气和营养支持治疗37例分析

目的探讨急性呼吸窘迫综合征（ARDS）患者机械通气和营养支持治疗的临床特点及治疗效果。方法回顾性分析37例ARDS患者经机械通气和营养支持治疗的临床资料。结果37例ARDS机械通气患者1次脱机成功者30例，2次脱机成功者5例，3次脱机成功者2例。机械通气3—10d，无一例依赖呼吸机。全胃肠外营养（TPN）支持4-12d，TPN支持期间合并高血糖5例，消化道出血4例，肺感染、呼吸衰竭5例，无一例中心静脉置管并发症。结论机械通气和营养支持治疗ARDS临床效果较好。但应注意营养供给方式的选择。

Mechanical ventilation and nutritional support for the treatment of 37 patients with acute respiratory distress syndrome

Objective To investigate the clinical features and effect of the treatment of mechanical ventilation and nutritional support for patients with acute respiratory distress syndrome （ARDS）. Methods The clinical data of 37 cases of ARDS treated with mechanical ventilation and nutritional support were retrospectively analyzed. Results Of the 37 patients who underwent mechanical ventilation, initiation of spontaneously breathing occurred in 30, 5 and 2 patients at the first, second and third off - line performance respectively. The duration of mechanical ventilation in these patients was 3 - 10 days and no patient was found ventilator - dependent. The patients received total parenteral nutrition （TPN） support for 4 - 12 days. High blood sugar, gastrointestinal bleeding, lung infection and respiratory failure were found in 5, 4 and 5 patients respectively during the period of TPN support. No central venous catheter complications were noticed in these patients. Conclusion The mechanical ventilation and TPN support show good effect for the treatment of ARDS, but attention should be paid to the performance of nutrition supply.
