肝外恶性梗阻性黄疸214例B超检查与手术所见对照分析

目的:从外科临床角度评价B超在肝外恶性梗阻性黄疸鉴别诊断中的作用。对象与方法:1985年～1998年来本院治疗的214例肝外恶性梗阻性黄疸病人术前全部行B超检查,并均经手术证实诊断。结果:本组211例病人术前超声检查发现有胆管扩张,据此诊断为肝外梗阻性黄疸,其正确率为98.6％。判定梗阻部位的正确率为93.5％。结论:术前B超检查对黄疸性质及梗阻部位可提供确切的诊断依据,但对梗阻的性质仍不满意,需进一步研究或辅以其它检查。

Comparative Analysis between Ultrasonography and Operation in patients with Extrahepatic Malignant obstructive Jaundice.

Purpose: To evaluate use of B model ultrasonography in differential diagnosis of extrahepatic malignant obstructive jaundice. Materials and Methods: 214 patients with extrahepatic malignant obstructive jaundice, who was collected from 1985 to 1998 years, underwent B model ultrasonography before the operation and obtained demonstration by postoperative pathology. Results: Expansions of biliary ducts were found in 211 patiants (98. 6% ) by preoperative ultrasonography, based on these findings, these patients were diagnosed as extrahepatic malignant obstructive jaundice. The diagnostic accuracy was 98. 6% but the accuracy to identify the obstructive position was 93.5% . Conclusions: Preoperative ultrasonography in patients with obstructive jaundice can provide reliable evidences for determining the properties of jaundice and the position of obstruction but the differentiation of obstructive properties is not still satisfactory, therefore, it needs further study or to combine with other examinations.
