主动脉内球囊反搏在高危患者行PCI的应用体会

目的：探讨伴有心源性休克的高危急性心肌梗死患者在主动脉内球囊反搏（IABP）支持下行直接PCI的疗效。方法：选取2009年12月-2012年12月本院收治的53例高危急性心肌梗死患者，均接受IABP辅助治疗直接行PCI。比较IABP使用前后临床观察指标的变化情况。结果：IABP使用后患者平均动脉压升高，多巴胺用量减少，心电图缺血表现改善，平均心率下降，7 d内的血浆脑钠肽（BNP）水平下降。上述指标与IABP使用前比较差异均具有统计学意义（P〈0.05）。结论：伴有心源性休克的高危急性心肌梗死患者在IABP支持下行直接PCI可明显改善心脏功能，降低死亡率，具有广阔的应用前景，值得进一步推广。

The Experience of Intra-aortic Balloon Pump Application for High-risk Patients Undergoing PCI

Objective：To investigate the efficacy on directly undergoing PCI under the support of aortic balloon pump in patients with high-risk acute myocardial infarction accompanied with cardiogenic shock. Method：Selected 53 patients in our hospital with high-risk acute myocardial infarction from December 2009 to December 2012,accepted intra-aortic balloon pump adjuvant therapy directly undergoing PCI .Compared the hemodynamic index changes before and after used IABP. Result：After the use of IABP the mean arterial pressure of high-risk patients with acute myocardial infarction elevated,dopamine dosage reduced,ecg ischemia performance improved,the average heart rate declined,the plasma brain natriuretic（BNP）levels dropped in 7 days. Compared with before use of IABP,those indexs changes had significant difference（P〈0.05）.Conclusion：High-risk patients with acute myocardial infarction associated with cardiac shock,within the aortic balloon pump support directly undergoing PCI can significantly improve the function of heart,reduce mortality,has a broad application prospect,is worthy of further promotion.
