创伤性体表器官缺损的美容整复

摘    要:目的:以不同的皮肤组织瓣移植修复再造创伤性体表器官。方法:用扩张后皮瓣,颈胸皮瓣、颧部皮瓣、前臂桡动脉皮瓣、颞浅筋膜瓣、背阔肌皮瓣,腹直肌皮瓣移植来修复再造颜面颈部深度缺损、耳廓缺损、眼睑外翻缺损、乳房缺如和阴茎缺损等。结果:治疗131例,组织瓣移植成活和器官再造外形美观的129例,1例洞穿性鼻缺损扩张后皮瓣再造部分坏死,1例乳癌根治后Ⅰ期腹直肌皮瓣再造乳房全部失败。随防半年以上的病例82例,外形均与正常相近。结论:创伤性体表器官缺损的程度形状各异,而且多数缺损处及周围都有大量的瘢痕组织增生,修复也应根据具体情况选择适应的组织瓣再造,才能使外观更加逼真美观,使已创伤体表器官得以完美的恢复。

Abstract:Objective To repair and reconstruct the traumatic organ defect on body surface with different skin flaps. Methods The expansion skin flap, cervicothoraces skin flap, zygoma skin flap, radial forearm skin flap, temporal fascia flap,latissimus dorsi musculo cutaneous flap and rectus abdominis musculo cutaneous flap were used to repair the deep defects on the face and neck,and reconstruct the traumatic defect of nose, auricle, breast and penis. Results Of 131 cases,129 cases of the skin flaps were survived, the focal necrosis appeared in an expansion skin flap used to repair nose defect,and the total necrosis occurred in an rectus abdominis musculo cutaneous flap to reconstruct the breast defect after radical mastectomy.The follow-up results from 82 cases over six-months showed the function and appearance of the reconstructed organs were satisfactory. Conclusion The degree and shape of the traumatic organ defect on body surface are very different and there are a lot of cicatrix around focus. According to different defect to select a suitable skin flap to repair it, the more beautiful and lifelike appearance will be achieved.
