摘    要:目的 探讨血管源性因子对妊娠子痫前期发病预测的价值.方法 采用酶联免疫吸附试验检测可溶性内皮糖蛋白(sEng)、可溶性血管内皮生长因子-1(sFlt-1)、胎盘源性生长因子(PLGF)在正常妊娠妇女、轻度子痫前期患者及重度子痫前期患者血清中的水平,探讨三者对子痫前期发病预测的价值.结果 重度子痫前期组sEng(7148±802)pg/ml、sFlt-1(2019±383)pg/ml,明显高于轻度子痫前期组和正常妊娠组(P均<0 01).结论 血循环中的血管生成因子平衡失调,可能在子痫前期的发病机制中起主要作用,联合测定sEng、PLGF、sFlt-1,有望成为预测和早期诊断子痫前期的有效方法并应用于临床.

Abstract:Objective To investigate the changes in circulating biomarkers of angiogenic activity before and after the onset of preeclampsia. Methods Serum concentrations of placental growth factor ( PLGF), soluble fins - like tyrosine kinase 1 ( sFh - 1 ), and soluble endoglin were prospectively measured from 10 weeks gestation until 12 weeks postpartum in 67 pregnant women with preeclampsia (including mild and severe preeclampsia) and 39 normal pregnant women were determined by enzyme linked immunosorbent assay. Results The women who developed severe preeclampsia had lower serum PLGF ( P = 0. 003 ) , higher soluble endoglin ( P = 0. 006), and higher sFh - l : PLGF ratio ( P : 0. 005 ) compared with women who later developed mild preeclampsia, or normotensive pregnancy. Conclusion These observations support a role for placenta - derived angiogenic biomarkers in the pathogenesis of preeclampsia. Combined determination tbe vascular endothelial growth factor (VEGF), placenta growth factor(PLGF), soluble VEGF receptor 1 ( sFlt - 1 ) is expected to become an effective method for predicting and early diagnosis of preeclampsia.
