胰腺损伤的内镜诊断与治疗

目的 探讨内镜在胰腺损伤诊断与治疗中的价值.方法 回顾性分析2007年1月至2010年12月期间作者收治的7例胰腺损伤患者的临床资料,其中6例患者行ERCP检查,确诊主胰管破裂4例,主胰管完好2例.结果 5例患者采取保守治疗;1患者采取手术治疗.结论 ERCP对胆胰系统的损伤能提供精确诊断,有利于制定合理的治疗计划,通过放置胰管支架或鼻胰管引流,使部分患者免于手术.

Clinial study on the role of endoscopic retrograde cholangiopancreatography in the diagnosis and treatment of pancreas injury

Objective To evaluate the role of endoscopic retrograde cholangiopancreatography （ERCP） in the diagnosis and treatment of pancreas injury. Method The clinical data of 6 pancreas injury patients were collected and analyzed retrospectively. All patients were admitted and treated at the department of general surgery, Xinhua hospital from Jan 2007 to Dec 2010. ERCP were performed for 6 of the patients. Among them, 4 were confirmed to have pancreatic duct injury and the other two showed no signs of pancreatic duct injury. Result 5 patients were treated conservatively. The anther patient had naso - pancreas draining tube placed by ERCP after splenectomy and partial pancreas resection. No mortality occurred in our study. Conclusion ERCP is effective for accurate diagnosis of pancreas injury and it will also be beneficial for the planning of treatment strategy. With the assistance of ERCP, a stent or a draining tube could be used in some patients to exempt them from open surgery.
