食管结核诊治10例报告

目的　增强临床工作者对食管结核的认识 ,以提高诊疗水平。方法　回顾分析 1 972 - 1 997年收治的 1 0例食管结核病例的临床特点。结果　食管结核缺乏特异性的症状体征 ;以往的文献往往夸大结核全身症状及既往结核病史对本病诊断的意义。在食管造影上本病不易与食管癌或食管平滑肌瘤鉴别 ;胸部CT在诊断上有重要意义。食管镜对食管结核的诊断率不高 ,但其意义在于除外食管癌。正规抗痨治疗 1个月 ,多数病例的症状及X线表现能得到明显改善。结论　综合分析病史、影像及内镜特点 ,多数病例能做出明确诊断 ;诊断性治疗是一个重要补充手段

Diagnosis and treatment of 10 cases of esophageal tuberculosis

Objective\ To describe the characteristics of esophageal tuberculosis(ESTB).Methods\ The clinical characteristics of 10 patients with ESTB were analyzed retrospectively. Results\ No specific symptoms or signs were identified. General symptoms of tuberculosis and past tuberculosis history were of less importance for the diagnosis of ESTB. It was very difficult to differentiate ESTB from esophageal carcinoma or leiomyoma on esophagography;the chest CT was of great value for the differentiation. Esophagoscopy had little value to reach a definite diagnosis, but could exclude cancer. The symptoms and the esophagographic abnormalities could be improved significantly by a one-month-term anti-tuberculosis therapy. Conclusion\ The combination of the analysis of case history, the findings of roentgenography, as well as endoscopy could lead to a definite diagnosis in most cases,in which the diagnostic anti-tuberculosis treatment was an important supplemental aspect.
