压力调节容量控制通气在救治老年危重支气管哮喘患者中的应用研究

目的探讨压力调节容量控制通气（PRVC）模式治疗老年危重支气管哮喘的效果。方法53例老年重症哮喘患者被随机分为A组（28例）和B组（25例），A组予PRVC通气，B组予SIMV＋PSV，比较两组患者治疗前后的动脉血气分析（ABGs）、呼吸力学、通气时间及住ICU时间。结果A组患者在使用MV后，ABGs明显改善（P〈0．01），PIP、Pplat较B组降低，Cdyn优于B组，MV时间和住ICU时间均短于B组（P〈0．05）。结论PRVC在治疗老年重症哮喘患者时，能够保持较低的PIP和Pplat，改善ABGs和Cdyn缩短MV时间以及住ICU时间，是一种安全而有效的通气模式。

A study on the treatment of severe asthma with pressure regulated volume control ventilation in elderly patients

Objective To evaluate the effect for the treatment of severe asthma with pressure regulated volume control（PRVC） ventilation in elderly patients. Methods 53 elderly patients with severe asthma were divided into two groups randomly, group A （ n = 28 ） were treated with PRVC, group B （ n = 25 ） were treated with SIMV plus PSV. The ABGs, respiratory dynamics, time of mechanical ventilation and ICU stay were measured. Results After mechanical ventilation, the ABGs was improved obviously in group A （ P 〈 0. 01 ） ; PIP and Pplat was lower, Cdyn was better, time of mechanical ventilation and ICU stay was shoter in group A than that in group B （ P 〈 0. 05 ）. Conclusion PRVC mode can decerese PIP and Pplat ,improve ABGs and Cdyn, shorten the time of mechanical ventilation and ICU stay. It is safe and effective for elderly patients with severe asthma.
