经尿道前列腺电汽化切割术疗效影响因素再分析

目的分析经尿道前列腺电汽化切割术（TUVP）疗效影响因素，提高TUVP质量。方法分析306例前列腺增生（BPH）患者TUVP资料。术前B超测定前列腺重量25-140g，平均62．5g，最大尿流率（Qmax）平均7．4mL／s。平均尿流率（Qave）平均3．9mL／s，残余尿量（RU）平均80．2mL。结果306例患者手术效果基本满意，切除前列腺组织平均50g。电汽化时间平均65min，291例（95．1％）患者随访3—40个月，Qmax平均增至18．1mL／s，Qave平均增至11．2mL／s，RU平均降至13mL。各项指标与术前比较，差异均有统计学意义（P〈0．01）。结论术前、术中和术后仔细分析TUVP疗效影响因素，使操作规范化，可提高TUVP质量。

Re-analysis of factors influencing therapeutic efficacy of transurethral electrovaporization ablation of prostate

[Objective] To re-analyze the factors which influence the therapeutic efficacy of transurethral electrovaporization ablation of the prostate (TUVP) and to improve the qualities of TUVP. [Method] The data of 306 patients with benign prostatic hyperplasis (BPH) were studied. Preoperatively, the wage of the prostate was 25~140 g, with mean of 62.5 g, measured by B-ultrasound. The average maximum urinary flow rate (Qmax) was 7.4 mL/s, and average urinary flow rate (Qave) was 3.9 mL/s, the average residual urinary volume (RU) was 80.2 mL. [Results] TUVP was successfully performed on all the 306 patients. The average electrovaporization ablation time was 65 minutes. The average resected prostate tissue weight was 50 g. Of the 306 cases, 291 cases (95.1%) were followed up for 3~40 months. Postoperative average Qmax increased to 18.1 mL/s, Qave increased to 11.2 mL/s an RU decreased to 13 mL. Compared with those of pre-operation, the differences were statistically significant (P <0.01). [Conclusions] Careful analysis of the efficacy-influencing factors before, during and after operation can improve the qualities of TUVP for the patients with BPH.
