腰椎跳跃式爆裂性骨折的手术治疗

目的　探讨腰椎跳跃式爆裂性骨折的机制和手术治疗方式。方法　采用椎管间接减压和后路长节段椎弓根钉系统复位固定方法 ,治疗腰椎跳跃式爆裂性骨折 14例。结果　术后骨折椎体高度明显恢复 ,椎管狭窄改善 ,经 6个月～ 4年 2个月随访 ,复位后椎体高度无丢失 ,无后凸畸形 ,Frankel分级获得 1～ 3级以上的改善。结论　腰椎跳跃式爆裂性骨折严重影响腰椎的稳定性 ,需及时手术治疗 ,经后路长节段椎弓根钉系统复位固定和间接椎管减压 ,可有效重建脊柱稳定性 ,改善神经功能。

The Surgical Treatment of Lumbar Jumping Burst Fractures

Objective In order to research the mechanism and a better operative method for lumbar jumping burst fractures.Methods Fourteen patients with lumbar jumping burst fractures received long-segment transvertebral pedicle internal fixation and indirect posterior decompression.Results The height of each fracture vertebra was reducted and had hardly loss without kyphosis from 6 months to 4 years and 2 months followed-up. The spinal canal stenosis was notably improved. Nervous function had improved by one to three grades according to Frankel's grading.Conclusion The patient with lumbar jumping burst fractures had severe unstability in his lumber, immediate operation of transvertebral pedicle internal fixation and indirect posterior decompression is needed which can reconstruct the spinal stability and improve nervous function effectively.
