三步针罐疗法治疗腰椎间盘突出症影响疗效多因素分析

目的分析三步针罐疗法治疗腰椎间盘突出疗效的影响因素。方法选择经三步针罐疗法治疗的腰椎间盘突出症患者90例,分析其临床疗效,研究疗效与性别、年龄、病程、椎间盘突出位置、单或多间隙间盘突出、有无侧隐窝狭窄、有无椎管狭窄、患者是否配合卧床休息、初发或复发腰腿痛等因素的相关性,并对相关因素进行logistic线性多元回归分析。结果三步针罐疗法治疗的腰椎间盘突出症的疗效与年龄、椎间盘突出位置、有无侧隐窝狭窄、有无椎管狭窄成负相关(均P0.05),与是否配合休息、初发或复发腰腿痛成正相关(均P0.05)。结论年龄、椎间盘突出位置、有无侧隐窝狭窄、有无椎管狭窄、是否配合休息、初发或复发腰腿6个因素在针刺治疗腰椎间盘突出疗效中起着重要的作用。

Multifactorial Analysis of the Therapeutic Efficacy of Three-step Acupuncture-cupping Therapy in Treating Lumbar Intervertebral Disc Herniation

Objective To analyze the influential factors in the treatment of lumbar intervertebral disc herniation （LIDH） with three-step acupuncture-cupping therapy. Method Ninety LIDH patients treated by the three-step acupuncture-cupping therapy were chosen for analyzing the therapeutic efficacy and studying the relationship between the efficacy and a variety of factors, including gender, age, disease duration, location of the herniation, single or multiple herniation, with or without lateral recess stenosis, with or without spinal stenosis, bed rest, and initial attack or relapsed attack of lumbago. The analyses were conducted by using multiple logistic regression. Result The therapeutic efficacy of three-step acupuncture-cupping therapy in treating LIDH is negatively related to age, location of herniation, lateral recess stenosis, and spinal stenosis （P〈0.05）; it＇s positively related to bed rest and initial or relapsed attack of lumbago （P〈0.05）. Conclusion Six factors, including age, location of herniation, lateral recess stenosis, spinal stenosis, bed rest, and initial or relapsed attack of lumbago, are playing crucial roles in the treatment of LIDH with acupuncture.
