腹腔镜下筋膜内子宫切除术128例临床分析

目的探讨腹腔镜下筋膜内子宫切除术（CISH）的临床应用价值。方法我院自2003年以来开展腹腔镜下筋膜内子宫切除术128例，随机抽取同期腹腔镜下全子宫切除术（TLH）115例进行比较。结果两组在手术时间、术后出血比较无显著性差异（P〉0．05）。而CISH组术中出血量、住院天数、术后最高体温、肛门排气时间、中转开腹与TLH组比较差异有显著性（P〈0．05）。结论腹腔镜下筋膜内子宫切除术损伤小，术中出血少，并发症少，术后恢复快，是一种较理想的手术方式，值得临床推广应用。

Clinical Analysis of 128 Cases of Classical Intrafascial Supracervical Hysterectomy under Laparoscope

Objective To discuss the clinical application of classical intrafascial superacervical hysterectomy （CISH） under laparoscope. Methods 128 cases have been treated with CISH since 2003 in our hospital,and a contrast study was made between them and 115 cases treated with total laparoscopic hysterectomy （ TLH ） selected randomly during the same period. Results There existed no obvious difference in operation duration and blood loss after operation between the 2 groups while the bleeding volume in operation, days of hospitalization, the highest temperature after operation, the time of anal exsufflation and conversion to open surgery of CISH group were quite different from those of TLH group （ P 〈 0. 05 ）; Conclusions Classical intrafascial supracervical hysterectomy under laparoscope is of such advantages as smaller trauma,less bleeding in operation,less complications and quick recovery after operation,and it is worthy of wide clinical application.
