食管癌术后并发心律失常相关因素的临床分析

目的探讨食管癌术后发生心律失常的相关因素。方法分析2001年5月-2008年2月间734例食管癌术后患者的资料。结果128例发生各种类型心律失常。年龄／〉70岁、术前心律失常、术前合并高血压者、术前经放化疗后尤其术前经同期放化疗后，FEV1／FVC〈70％以及手术时间〉5h，颈部吻合、主动脉弓上吻合，术后低钾、低氧，酸中毒，无镇痛者，其术后发生心律失常的几率显著增加（P〈0．01）。结论食管癌术后心律失常的发生率与患者的年龄、FEV1／FVC〈70％、术前心律失常、术前合并高血压者，术前经放化疗及手术时间〉5h，食管胃吻合的部位，术后低钾、酸中毒、低氧、无镇痛密切相关。

Clinical analysis of correlative factors of postoperative arrhythmias in esophageal carcinoma

Objective To investigate the correlative factors of postoperative arrhythmias in esophageal carcinoma. Methods From May,2001 to Feb,2008,734 postoperative patients with esophageal carcinoma were studied. Results one hundred and twenty-eight patients had postoperative arrhythmias. The analytics showed the patients who were older than 70 years, had pre-operative arrhythmias, had pre-operative hypertension and had pre-operative radiation therapy or chemotherapy. especially had pre-operative radiation therapy combine chemotherapy. FEV1/FVC<70%,the operation time longer than 300 minutes,cervical esophago-gastroplasty,esophago-gastroplasty over aortic arch, had postoperative hypopotassemia,hypoxemia,acidosis and who had no ease pain were easer to suffer from postoperative arrhythmias(P<0.01). Conclusion Postoperative arrhythmias in esophageal carcinoma are significantly correlate with age of patient, FEV1/FVC<70%, pre-operative arrhythmias, pre-operative hypertension, pre-operative radiation therapy or chemotherapy, the operation time longer than 300 minutes,the position of esophago-gastroplasty, postoperative hypopotassemia,hypoxemia,acidosis and no relieve pain after operation.
