血清降钙素原在老年患者急性感染性疾病鉴别诊断中的临床意义

目的 探讨降钙素原（PCT）对老年患者急性感染性疾病鉴别诊断的临床意义。方法 将198例老年患者分成细菌感染组和病毒感染组并进行PCT检测，与血清C-反应蛋白（CRP）水平作比较，同时将106例细菌感染组患者分为重症感染、一般感染组并与健康对照组进行分析比较。结果 重症感染组PCT阳性率91．18％，一般感染组阳性率为62．28％，健康对照组阳性率为9．09％，3组间差异有统计学意义（P〈0．01）;以0．5ng／ml为阳性阈值，在重症感染时PCT的敏感度为91．18％、特异性为57．50％：以2．0ng／ml为阳性阈值，在重症感染时PCT的敏感度为61．76％、特异性为84．17％，与CRP相比，其敏感度和特异性更高。结论 在急性重症细菌感染的老年患者中血清PCT的检测比CRP更有鉴别诊断价值。

Clinical significance of serum procalcitionin determination in old patients with acute infection disease

Objective To evaluate clinical diagnosis value of procalcitionin (PCT) in old patients with acute infectious disease. Methods 198 old patients with acute infectious disease were divided into bacterial infection group and viral infection group. Their serum PCT were measured and compared with C-reaction protein (CRP) levels .106 old patients with bacterial infection were divided into serious infection group and ordinary infection group,The results of two groups were compared with the control group. Results The positive rate was 91.18%in serious infection group, 62.28%in ordinary infection group and 9.09%in control group and there were statistic significance( P<0.01).If regarded 0.5ng/ml as positive threshold, the sensitivity of PCT in serious infection group was 91.18%and specificity was 57.50%; If regarded 2.0 ng/ml as positive threshold, the sensitivity PCT was 61.76%and its specificity was 84.17%. Sensitivity and specificity of PCT were higher compared with CRP. Conclusion Serum PCT have higher identified diagnosis values in acute serious bacterial infection of old patients compared with CRP.
