血浆脑钠肽对呼吸困难病因的鉴别诊断价值

目的探讨脑钠肽（brain natriuretic peptide,BNP）对心源性和肺源性呼吸困难的鉴别价值。方法用酶联免疫吸附法测定126例心源性和肺源性呼吸困难患者血浆BNP浓度,同时检测50例健康体检者的BNP水平作为对照。结果心源性呼吸困难组BNP浓度为（1103±902）ng/L,显著高于肺源性呼吸困难组（85±21）ng/L和健康对照组（41±15）ng/L（P〈0.01）;心源性呼吸困难组BNP水平在不同心功能级别间有显著差异。结论BNP可作为鉴别心源性呼吸困难和肺源性呼吸困难的一个客观指标，其方法简单快速，具有显著的临床意义。

Value of Serum Brain Natriuretic Peptide Levels Determine in Differential Diagnosis

Objective To investigate the value of brain natriuretic peptide in the differential diagnosis of dyspnea caused by heart disease or pulmonary disease.Methods Enzyme-linked immunosorbent assay method was used to measure the levels of serum BNP in 58 patients with congestive heart failure（CHF）,68 patients of pulmonary dyspnea without CHF,and 45 healthy subjects serving as control group.Results The levels of serum BNP in CHF group,dyspneic group and control group were（1 103±902）,（85±21） and （41±15）ng/L respectively.The levels of serum BNP were much higher in CHF group than those of dyspnea group and control group（P〈0.01）.The level of BNP was significantly different at their different stages of heart function in CHF group.Conclusion BNP,which has remarkable clinical significance,can be used as the indicator in the differential diagnosis of dyspnea
