急性缺血性脑卒中OCSP分型与磁共振成像分型的对应关系

摘    要:目的探讨急性脑梗死牛津郡社区卒中项目(OCSP)分型与磁共振成像分型的对应关系。方法采用单盲法,回顾性比较新疆生产建设兵团医院神经内科脑卒中登记库2007年1月~2009年2月282例急性脑梗死患者早期OCSP分型情况及其磁共振成像分型特征。结果 OCSP分型结果显示,32例(11.3%)为完全性前循环梗死(TACI),86例(30.5%)为部分性前循环梗死(PACI),111例(39.4%)为腔隙性梗死(LACI),53例(18.8%)为后循环梗死(POCI)。核磁共振结果显示,201例患者的临床分型与影像学分型一致,分型总准确率为71.3%。各亚型分型准确率分别为TACI77%(27/35)、PACI79%(42/53)、LACI69%(95/137)、POCI65%(37/57)。结论 OCSP分型能正确预测梗阻部位和大小,与磁共振检查结果有良好的对应关系。

Abstract:Objective To investigate the relationship between Oxfordshire community stroke project(OCSP) classification and MRI classi cation in acute cerebral infarction.Methods A total of 282 patients with acute cerebral infarction were retrospectively evaluated with OCSP classification and imaging characteristics.Results According to OCSP classi cation,of all 282 patients with acute cerebral infarction,32(11.3%) experienced total anterior circulation infarction(TACI),86(30.5%) partial anterior circulation infarction(PACI),111(39.4%) lacunar infarction(LACI),and 53(18.8%) posterior circulation infarction(POCI).The consistency was found in 201 cases(71.3%) between the OCSP classi cation and imaging classification,with the accuracy of 77%(27/35) for TACI,79%(42/53) for PACI,69%(95/137) for LACI and 65%(37/57) for POCI.Conclusion OCSP classi cation can predict the location and size of cerebral infarction with a high accuracy,and is well consistent with the MRI ndings.
