TVS卵泡监测序贯尿LH试纸预测排卵在计划妊娠中的指导意义

摘    要:目的:探讨阴道超声(TVS)监测卵泡发育序贯尿黄体生成素(LH)试纸预测排卵的科学性和准确性及在计划妊娠中的指导意义。方法:选择有意向妊娠且无明显不孕因素的育龄妇女分为两组:A组:于自然月经第9~17天行阴道超声监测卵泡发育,隔日一次,追踪观察卵泡生长状况,至成熟、排卵为止;待发现优势卵泡近排卵期时,序贯采用尿LH试纸检测尿LH,每晨一次,呈阳性、达顶峰24 h内性生活,指导早日受孕;若阴性,再辅以血LH测定,预测排卵时间并了解卵巢功能。B组:随机抽取50例无任何监测方法的自然受孕妇女作对照。结果:A组46例共监测84个月经周期,B超监测到成熟卵泡67个周期,证实排卵周期63个,占75.0%;71个月经周期出现尿LH峰,阳性率为84.5%;两种监测排卵方法排卵周期符合率无统计学差异。46例中有15例在监测周期受孕,故监测周期的受孕率为32.6%(15/46),4~6个月间4例自然妊娠,自然妊娠率为8.7%。B组3个月内自然受孕4例,占8.0%,4~6个月间妊娠6例,受孕率为12.0%。两组3个月内受孕率差异有统计学意义(P<0.05)。结论:TVS监测卵泡生长直观、动态、科学;尿LH试纸检测简便易行,准确可靠,两者结合既能监测卵泡从生长至成熟卵泡的动态过程,又能预测排卵时间,为尽快受孕找到了最佳时机。因此,阴道超声监测卵泡发育序贯尿黄体生成素试纸预测排卵在计划妊娠中具有积极、科学的指导意义,并在优生优育领域起到重要作用。
