环磷腺苷注射液治疗扩张型心肌病重度心力衰竭28例临床观察

目的:观察环磷腺苷注射液治疗扩张型心肌病重度心力衰竭的疗效。方法:扩张型心肌病重度心力衰竭例58例,随机分为治疗组(28例),对照组(30例)。扩张型心肌病病例系按1995年世界卫生组织及国际心脏病学会(WHO/ISFC)定义选择入围。NYHA心功能达Ⅲ~Ⅳ级者入选,共58例,其中治疗组Ⅳ级18例,Ⅲ级10例,对照组Ⅳ级16例,Ⅲ级有14例。两组均予地高辛、速尿、安体舒通、卡托普利治疗,其中治疗组加用环磷腺苷注射液40 mg加入生理盐水或5%葡萄糖注射液250 ml静脉滴注,1次/d,10~15 d为一疗程。结果:治疗组总有效率78.6%,显著优于对照组的总有效率46.7%(P<0.05),且治疗组心脏左室射血分数改善(31.71%~43.29%)明显好于对照组(32.30%~37.43%),P<0.05。结论:在传统治疗基础上,加用环磷腺苷注射液能进一步改善扩张型心肌病重度心力衰竭的心功能,提高临床疗效。

Clinical effect of cAMP injection on patient with dilated cardiomyopathy complicated severe congestive heart failure

Objective:To observe the clinical therapeutic effect of cAMP injection on patient with dilated cardiomyopathy(DCM) complicated severe congestive heart failure(CHF).Methods:The 58 patients with DCM accompanied severe CHF were randomily diveded into treatment group(28 cases) and contrast group(30 cases).In the contrast group,conventional therapy such as digoxin,antisterone and captopril etc.were given,while in the treatment group, cAMP injection(40 mg/d) was given in addition to conventional therapy.Their therapy duration was 10 to 15 days.Results:The total effect rate(78.6%) and the improvement of left ventricular ejection fraction(31.71%~43.29%) was more than those of control group respectively(46.7%,32.3%~37.43%),P<0.05 all.Conclusion:The cAMP injection can improve the cardiac function and improved the clinical therapeutic effect in DCM patient with severe CHF.
