适形放射治疗老年原发性非小细胞肺癌的临床观察

目的 观察适形放射治疗在老年原发性非小细胞肺癌中的治疗效果.方法 对89例年龄≥70岁,KPS≥60的老年原发性非小细胞肺癌进行适形放射治疗,照射剂量为4 000～6 000 cGy,分次剂量400～500 cGy.对有纵隔淋巴结转移的患者先行原发灶、肺门及纵隔常规外照射,剂量为3 000～4 000 cGy.结果 所有患者均完成适形放射治疗,完全缓解(CR)45例,占50.6%,部分缓解(PR)31例,占34.8%,无缓解(NR)11例,占12.4%,总1、2、3年生存率分别是73.0%、41.6%、32.6%.治疗期间发生急性放射性肺炎11例(12.4%),Ⅰ、Ⅱ级放射性食管炎21例(23.6%),经对症支持治疗后均缓解.结论 适形放射治疗是一种提高老年原发性非小细胞肺癌患者生存率和生存质量的有效治疗手段.

Three-Dimmentional Conformal Radiotherapy for Non-Small Cell Lung Cancer in the Elderly

Objective To discuss the effect of three-dimmentional conformal radiotherapy on non-small cell lung cancer in the elderly. Methods 89 cases over 70-year-old patients with non-small cell lung cancer received three-dimmentional conformal radiotherapy at a dose of 4 000 cGy to 6 000 cGy, the cases with lymphnode metastatic received conventional radiotherapy at a dose of 3 000 cGy to 4 000 cGy. Re- sults All the cases received three-dimmentional conformal radiotherapy successfully. 1-,2- and 3-year survival rate were 73.0% , 41.6% , 32.6%. The incidences of acute radiation pnellmonitis and esophagitis were 12.4% and 23.6% ,which were cured by medication. Conclusions Three-dimmentional conformal radiotherapy for aged patients with lung cancer is a good therapeutic selection with improved survival quality and identical survival rate.
