不同护理方法对全身麻醉术后眼睛的影响

目的:观察不同护理方法对全身麻醉下眼睛保护的有效性和安全性。方法:选择择期在全身麻醉下行腹腔镜手术患者120例,随机分为四组各30例,A组在麻醉诱导前嘱其自然闭眼,全麻后无任何眼睛护理。B组在麻醉诱导后给患者双眼涂上适量金霉素眼膏,再轻轻合上双眼,使上下眼睑闭合。C组在麻醉诱导后用3 cm×5 cm切口敷料粘贴于患者双眼睑上,确保眼睑紧闭。D组在麻醉诱导后给患者双眼滴适量(3滴)妥布霉素地塞米松滴眼液,并用3 cm×5 cm切口敷料粘贴于患者双眼睑上,确保眼睑紧闭,手术结束时再滴3滴。记录手术结束后眼结膜是否充血水肿、血丝,术后48 h内眼部症状。结果:手术结束后A,B两组眼结膜充血水肿、血丝情况高于C,D两组。术后48 h内,A,B,C三组眼睛异物感反应发生率高于D组(P<0.05);A,B两组眼睛不适感发生率高于C,D两组(P<0.05)。结论:双眼滴适量妥布霉素地塞米松滴眼液并用切口敷料粘贴于患者双眼睑上,确保眼睑紧闭,能有效预防全身麻醉术后眼部并发症的反应。

Effect of different ways to eyes protect during operation with general anesthesia

Objective：To observe the efficacy and safety of eyes protect by different nursing comparison of ways during operation with general anesthesion.Methods：120 patients undergoing selective laparoscopic operation with general anesthesion were randomly divided into four groups with 30 cases in each group.Group A：natural closed eyes before anesthesia,without any eyes nursing after anesthesia.Group B：The eyes coated appropriate chlortetracycline eye ointment,and then close the eyes,so that the upper and lower eyelid closure.Group C：The eyes were pasted with 3 cm×5 cm dressings after anesthesia to ensure that the eyelids closed.Group D：The eyes were driped the right amount（3 drops） of tobramycin and dexamethasone eye drops after anesthesia,and then the eyes were pasted with 3 cm×5 cm dressings after anesthesia to ensure that the eyelids closed,and the eyes were driped eye drops（3 drops） at the end surgery.The eye conjunctiva symptoms were recorded after surgery and the eye symptoms：asymptomatic,foreign body sensation and discomfort.Results：After the surgery,the group A,group B of conjunctival hyperemia edema bloodshot situation were higher than group C and group D.Within 48 h after surgery,group A,group B and group C,three sets of eyes foreign body sensation reaction were higher than group D（P＆lt;0.05）.Group A and group B,two sets of eyes discomfort were higher than in group C and group D（P＆lt;0.05）.Conclusion：Eyes were driped the right amount of tobramycin and dexamethasone eye drops and paste the double eyelid to ensure that the eyelids closed with dressing,were effective in precaution keratitis reaction after general anesthesia.
