摘    要:目的:探讨MSCTA对肺周围病变侵犯段以下肺动脉的诊断价值。方法:对362例肺周围病变行16层螺旋CT肺动脉造影,对其中段以下肺动脉受侵犯且经病理证实的35例进行分析。结果:段以下肺动脉可疑侵犯5例(良性2例,恶性3例);轻度侵犯18例(良性1例,恶性17例);重度侵犯12例(良性0例,恶性12例)。肺动脉轻重度侵犯对恶性病变的诊断准确率达96.7%,但肺动脉侵犯程度与良恶性结果间无显著性差异(P〉0.05)。结论:16层螺旋CT肺动脉造影能很好地显示周围病变侵犯段或亚段肺动脉的部位和程度,对诊断恶性病变具有重要参考价值。

Abstract:Objective:To explore diagnostic value of 16-slice spiral cr in peripheral pulmonary lesions invading segmental and inferior segmental pulmonary arteries. Methods:Pulmonary angiography of 16-sllce spiral CT was performed in 362 eases with pulmonary peripheral lesions, and invaded grades of pulmonary arteries and analysis of pathologic results were performed in 35 cases with suspicions or idenlifted invasion and with pathologic results in segmental and inferior segmental pulmonary arteries. Recruits: The suspicions invasion of segmental and inferior segmental pulmonary arteries was in 5 cases (benign 2 cases, malignant 3 cases) ; Low-gade invasion was in 18 cases (benign 1 case, malignant 17 cases) ; High-grade invasion was in 12 cases (benign 0 case, malignant 12 cases). The accurate rate of diagnosis was 96.7% in malignant lesions invading pulmonary alrteries which were low-grade plus high-grade, but it was no remarkable difference in the degree of invading pulmonary arteries with the benign and malignant lesions ( P 〉 0.05). Conclusion: Pulmonary angiography of 16-slice spiral CT can satisfactorily reveal the site and degree of involvement of segmental and inferior segmental pulmonary arteries by peripheral lesions, and which provide valuable reference to diagnose malignant lesions.
