支原体对输卵管性不孕患者体外受精与胚胎移植结局的影响

目的 :探讨输卵管不孕患者中宫颈解脲支原体 ( UU)感染对体外受精与胚胎移植 ( IVF-ET)结局的影响。方法 :回顾性分析本中心于 1 999年 3月～ 2 0 0 0年 1月因输卵管因素不孕进行常规IVF-ET治疗的患者 ,据宫颈分泌物 UU培养的结果及药物治疗后的结果分为 :2 5 7周期的 UU阴性组 ( A组 )、3 6周期的 UU治疗后转阴组 ( B组 )和 5 0周期 UU治疗后仍阳性组 ( C组 )。比较各组间受精率、卵裂率、优质胚胎率、临床妊娠率、种植率、异位妊娠发生率和流产率 ,行统计学分析。结果 :三组间受精率、卵裂率、优质胚胎率、临床妊娠率、种植率和流产率差异无统计学意义 ( P>0 .0 5 ) ,但 C组异位妊娠发生率较 A和 B组高 ( P<0 .0 5 )。结论 :IVF-ET中 ,输卵管因素不孕患者的宫颈分泌物 UU阳性 ,异位妊娠的发生率有增高趋势 ,但对胚胎发育、临床妊娠率、种植率及流产率无显著影响 ,经药物治疗 UU转阴后则对 IVF的结局无影响。

Retrospective Study on the Effect of Ureaplasma Urealyticum on IVF Outcome of Woman with Tubal Infertility

Objective: To investigate the effect of Ureaplasma Urealyticum (UU) in the cervix on the IVF outcome of women with tubal infertile. Meth ods: A retrospective analysis was performed among patients with [C M(46]tubal infertile undergoing IVF-ET in our center during March,1999~Jan uary,2000. The IVF cycles of [WT5BZ]cases were divided into three groups according to UU culture from the cervixes o f female patients: 257 cycles as UU negative group (group A), 36 cycles as U U negative group after treatment (group B), 50 cycles as UU positive group after treatment (group C). Fertilization rate, cleavage rate, top quality embryo rate, clinical pregnancy rate, implantation rate, abortion rate and ecto pic pregnancy rate were compared among three groups. Results: Group A and group B have no significant differences, but group C has higher ectopic pregnancy rate (P<0.05) than other group. Conclu sion: In IVF-ET, UU culture positive in cervical mucus of women wi th tubal factors has no effect on development of embryo, clinical pregnancy rate , implantation rate and abortion rate,but ectopic pregnancy rate tend to inc rease. If UU turns negative after treatment, IVF outcome will not be affected.
