电烧伤后强化胰岛素治疗高血糖

摘    要:目的分析探讨强化胰岛素对患者电烧伤后高血糖的治疗效果。方法以2009年8月-2013年6月期间该院收治的40例大面积电烧伤后合并高血糖患者为研究对象,随机分为对照组和观察组,分别采取短胰岛素治疗和强化胰岛素治疗,对比两组治疗效果、胰岛素用量以及第一次术前占床天数。结果7d后观察组患者胰岛素用量、术前占床天数均少于对照组,但血糖基本维持在4.1—6.3mmol/L,降糖效果好于对照组。结论强化胰岛素对于重度电烧伤后高血糖具有明显的降糖效果,能够大大节省胰岛素用量并缩短第一次手术前占床天数,值得在临床广泛推广。

Abstract:Objective To investigate the treatment effect of intensive insulin on patients who suffered electric burn complicated by stress hyperglycemia. Methods 40 cases of patients with hyperglycemia after large area electric burn admitted in our hospital from August, 2009 to June, 2013 were selected as the subjects. And they were randomly divided into the control group and observation group, treated by short insulin therapy and intensive insulin therapy, respectively. The treatment effect, the dosage of insulin and the first preoperative bed days of two groups were compared. Results 7 days after the treatment, though the dosage of insulin and the first preoperative bed days of the observation group were less than those of the control group, the blood sugar basically maintained within the range of 4.1-6.3mmol/L, the effect of reducing blood sugar is better than that of the control group. Conclusion For patients with hyperglycemia after severe electrical burns, intensive insulin can significantly control the blood sugar, reduce the dosage of insulin and shorten the hospital days before the first surgery, which is worthy of clinical application.
