1例老年慢性阻塞性肺疾病患者的药学服务体会

目的:通过临床药师参与1例慢性阻塞性肺疾病患者的治疗实践,探讨临床药师在临床诊疗活动中进行药学服务的方法。方法:临床药师深入临床治疗活动中,对患者的治疗过程全程参与;对医师、护士、患者提供不同的药学服务,积极参与药物治疗方案的制定、更改和实施。结果:患者经医药护的紧密合作,在药物治疗过程中,未出现不良反应及并发症,缩短了患者住院时间,患者症状好转出院,患者对本次治疗活动非常满意。结论:临床药师参与临床治疗,为医护患提供药学服务;与医护患进行沟通,保证了药物治疗的安全、有效;促进了医患关系的和谐发展,体现了临床药师的价值。

Experience of pharmaceutical care in one elderly patient with chronic obstructive pulmonary disease

Objective：To explore a method of pharmaceutical services during clinical diagnosis of clinical pharmacists, based on experiences of pharmaceutical care in one patient with chronic obstructive pulmonary disease. Methods： The clinical pharmacists deeply participated in the clinical diagnosis, joined in the whole treatment process, offered the pharmaceutical services to the doctors, burses and patients, and actively took a role in the formulation, change and implementation of the medication schemes. Results： After the participation of the clinical pharmacists, there were no adverse reactions and complications during the medication, the hospitaliza- tion time was shortened, and the patients were effectively treated and satisfied with the treatment. Conclusions： The method that The clinical pharmacists participate in the clinical diagnosis and offer the pharmaceutical services for the doctors and nurses and communi- cate with them is safe and effective, can promote the development of the doctor-patient relation, and also indicates the values of the clinical pharmacist.
