摘    要:目的:探讨新生儿肺透明膜病(hyaline membrane diseaas,HMD)X线影像特征。方法:对扬州市妇幼保健院新生儿病房42例确诊为HMD的患儿床边X线胸片进行分析,根据肺泡萎陷程度,HMD分为四级。结果:I级X线表现肺野透亮度略低,肺野内见细小颗粒影;II级X线表现肺野透亮度进一步降低,呈磨玻璃样改变,肺野内见密集分布均匀的细颗粒影,并见支气管充气征。III级X线表现肺内细颗粒影融合变大,边缘模糊,肺野透亮度明显降低,心膈模糊。IV级X线表现肺野密度均匀致密呈"白肺",心缘膈面完全消失。结论:HMD的X线影像具有特征性表现,X线胸片是诊断HMD的可靠检查方法。

Abstract:Objective:To discuss X-ray image features of neonatal pulmonary hyaline membrane disease (hyaline membrane diseaas, HMD). Methods :42 cases in neonatal ward of our hospital and diagnosed as HMD were selected, and their bedside X-ray films were analyzed. According to the degree of alveolar collapse, HMD were divided into four levels. Results: The lung field X-ray image of the I level HMD showed a slightly low transparency, and tiny particle shadows were see. The transparency of the lung field in those with II level HMD was further reduced and showed a ground-glass opacity, there were densely and homogeneously distributed fine particle shadows in the lung field, and an air-filled bronchi also could be seen. The X-ray image of III level HMD showed fused fine particle shadows in the lung with a blurry edge, the lung field transparency was significantly reduced, and also the heart diaphragm was blurry. The X-ray image of III level HMD showed a "white lung" with an uniform density, and the diaphragmatic surface of the cardi- ac bonder completely disappeared. Conclusions: The X-ray image of HMD has the characteristic features, and the X-ray chest radio- graph is a reliable examination method in diagnosis of HMD.
