阿立哌唑、利培酮和氯氮平对认知功能影响的比较研究

目的:比较抗精神疾病药物阿立哌唑、利培酮和氯氮平对精神分裂症患者认知功能的影响。方法:将符合诊断标准的精神分裂症患者90例随机分成三组,即阿立哌唑组、利培酮组和氯氮平组,单一用药,在4周、8周和12周通过用阳性和阴性症状量表(PANSS)评定三种药物的疗效,用韦氏成人智力量表(WAIS-RC)、临床记忆量表(CMS)评定患者的认知功能。比较三种抗精神病药物用药前后对认知功能的影响,并用统计学的方法给予定量分析比较。结果:12周PANSS总分表治疗前后比较呈显差异P<0.05;三组间比较无差异P>0.05;WAIS-RC总分治疗前后比较有统计学差异P<0.05;三组间比较有统计学差异P<0.05;阿立哌唑可改善患者的认知功能,CMS总分与利培酮和氯氮平相比P<0.05,能更好的改善患者的认知功能。结论:阿立哌唑对认知功能的影响小于利培酮和氯氮平。

Comparative study of effects of aripiprazole,risperidone and clozapine on cognitive function of patients with schizophrenia

Objective ： To compare effects of aripiprazole, risperidone and clozapine on cognitive function of patients with schiz- ophrenia. Methods ：90 patients meeting the diagnostic criteria of schizophrenia were randomly divided into aripiprazole group, risperi- done group and clozapine group, and they were treated only with one drug. 4, 8, and 12 weeks later, positive and negative symptoms scale （PANSS） was used to assess the curative effect, and wechsler adult intelligence scale （WAIS-RC） and clinical memory scale （CMS） were used to evaluate the cognitive function. The effects of aripiprazole, risperidone and clozapine on the cognitive function were compared before and after the treatment. Further, the statistical method was used to quantitatively analyze and compare, Results： 12 weeks later, the differences of PANSS scores before and after the treatment were significant （P〈0.05）, and there were no differ- ences among the three groups （ P〉0.05 ） ; and the differences of WAIS-RC scores before and after the treatment were significant （ P〈 0.05 ）, and there were the differences among the three groups （ P〈0.05 ）. Aripiprazole could improve the cognitive function of the pa- tients better than risperidone and clozapine, when comparing the CMS total scores （ P〈0.05 ）. Conclusions ： The effects of aripiprazole on the cognitive function is less than those of risperidone and elozapine.
