摘    要:目的 探讨梗阻性黄疸时肾脏功能与血浆一氧化氮(NO)水平及肾内诱生型一氧化氮合酶(iNOS)表达之间的关系。方法 用胆总管结扎法建立大鼠梗阻性黄疸模型,模型制备成功后2周分别检测正常组(N组,n=10)、胆总管结扎组(BDL组,n=20)和N^G-硝基L精氨酸甲酯组(L-NAME组,n=20)大鼠血浆胆红素、尿素氮、肌酐和NO水平,免疫组织化学染色法观测肾内血浆胆红素无明显变化,尿素氮、肌酐与NO水平均明显下降,与正常组水平相似,其间无明显差别,肾内iNOS表达亦不明显。结论 梗阻性黄疸时肾功能明显受损,血浆NO水平增高,肾内iNOS表达增加,NO在肾功能损害的发生中起一定作用,一氧化氮合酶抑制剂对梗阻性黄疸肾功能损害有保护作用。

Abstract:Objective To study the relationship of renal function in obstructive jaundice with plasma nitric oxide(NO) and the expression of renal inducible nitric oxide synthase(iNOS). Methods Fifty rats were randomly divided into normal group(N, n =10), common bile duct ligation group(BDL, n =20) and L NAME treated group(L NAME, n =20). The model of obstructive jaundice was established by ligation of common bile duct.The plasma nitric oxide, blood urea nitrogen(BUN),creatinine(Cr) and bilirubin were determined 10 d after operation. The immunohistochemical staining was used to detect the expression of inducible nitric oxide synthase in kidney. Results In BDL group the plasma bilirubin, BUN, Cr and NO were higher ( P <0.01) and the expression of iNOS was positive. The plasma bilirabin with no marked change, but BUN, Cr and NO were decreased obviously in L NAME group and similiar to that of control group. The expression of iNOS was not remarkable. Conclusion The increased NO and iNOS may play a role in obstructive jaundiced renal dysfunction.
