摘    要:目的检测肝移植术后患者感染的细菌标本中整合子的分布及其与耐药的相关性。方法收集27株肝移植术后分离的革兰阴性杆菌,鉴定并用纸片扩散法做药敏试验;聚合酶链反应(PCR)法检测整合酶及整合子基因盒,对阳性PCR产物采用Hinfl限制性片段长度多态性(RFLP)分析。结果27株细菌均为多重耐药,对9种抗生素耐药率从14.8%到100%;I型整合子阳性率为51.9%;有750bp、1.0kb、1.8kb、2.0kb 4种长度的基因盒片段。结论I型整合子广泛存在于多重耐药的肝移植术后患者感染标本,建议检测整合子,可用于防治院内感染、指导抗生素的合理使用。

Abstract:Objective To investigate the relativity between the distribution of integron and the integron-mediated multidrug resistance (MDR) in clinical isolates from the patient after liver transplantation. Methods 27 strains of gram-negative bacilli were collected from patients after liver transplantation. Antibiotic sensitivity was detected by disk diffusion on Mueller-Hinton agar. lntegron and inserted gene cassettes were amplified by polymorase chain reaction (PCR) and the positive PCR product was analyzed by restriction fragment length polymorphism(RFLP). Results 27 strains of them displayed MDR. The antibiotic-resistance rates ranged from 14.8% to 100%. The positive rate of class I integron was 51.9%. Integron cassettes with sizes of 750 bp, 1.0 kb, 1.8 kb and 2.0 kb were found. Conclusion The MDR are widely associated with the class I integron. Screening for integron plays an important role in prevention of hospital onset of infection and rational instruction of antibiotics usage.
