全肺切除治疗中心型非小细胞肺癌

目的]总结全肺切除治疗中央型非小细胞肺癌的临床结果。探讨心包内处理肺血管的技术要点。方法]回顾性分析浙江省肿瘤医院：1993年～1998年间85例全肺切除治疗的中央型非小细胞肺癌。结果]85例全肺切除病例，36例行心包内处理肺血管全肺切除。全组病例1、3、5年生存率分别为77．60％、36．47％、24．70％。其中姑息性全肺切除10例。无1例存活超过3年。结论]在肺功能良好的情况下，全肺切除及心包内处理肺血管全肺切除是一种安全可靠的手术方式．可提高晚期肺癌患者手术切除率、延长术后生存期。

Pneumonectomy for Treatment of Central Type Non-small Cell Lung Cancer

To summarize the clinical results of pneumonectomy for treatment of central type non-small cell lung cancer and to explore the techniques of intrapericardial pneumonecto-my.Retrospective study was carried out in this study to analyse85patients with central type non-small cell lung cancer undergoing pneumonectomy in Zhejiang Cancer Hospital from1993to1998.Among85patients undergoing pneumonectomy,36patients were dealed with major pulmonary vessels inside the pericardium.The1-,3-,5-year survival rates were77.60%,36.47%,24.70%respectively.Ten cases underwent palliative resection and none of them survived more than3years.Conclusion]In patients with good pulmonary functions,pneumonectomy and in-trapericardial pneumonectomy are safe and reliable,which increases resection rate of advanced lung cancer and improves the survival of patients.
