帕罗西汀联合利培酮治疗急性期精神分裂症伴发抑郁的疗效观察

目的观察帕罗西汀联合利培酮治疗精神分裂症急性期伴发抑郁症患者的临床效果和安全性。方法将100例急性期精神分裂症伴抑郁的患者随机分为对照组和联合组，对照组患者使用利培酮治疗，联合组使用帕罗西汀联合利培酮进行治疗，治疗周期为8周。使用简明精神病评定量表（BPRS）、汉密尔顿抑郁量表（HAMD）的17项标准以及不良反应症状量表（TESS）对两组患者进行症状及不良反应的评定。结果两组患者在服药后与服药前相比均表现出有益的影响，对照组患者的HAMD评分在第4周有显著性下降。联合组患者HAMD评分在治疗第1周时便有显著性下降，到治疗第2周开始也明显低于对照组。联合组BPRS评分在治疗第2周开始即有明显的降低，同时与同期对照组相比也有显著下降。治疗结束时，对照组痊愈率为20％，显著低于联合组的40％（P〈0．05），联合组的显效率略高于对照组。两组TESS评分无显著性差异。结论帕罗西汀联合利培酮治疗急性期精神分裂症伴发抑郁症患者的疗效优于单用利培酮，且不增加患者的不良反应。

Effect of paroxetine combined risperidone on acute schizophrenia patients complicated with depression

Objective To observe the outcome and safety of paroxetine combined risperidone on acute schizophrenia patients complicated with depression. Methods 100 acute schizophrenia patients complicated with depression were divided into control group and combined group, patients received risperidone or risperidone combined paroxetine for 8 weeks respective- ly. The indexes of BPRS score, HAMD score and TESS score were determined. Results All the patients exhibited benifitial effects after administration of each agent. The HAMD score in control group was ameliorated in the 4th week and that in combine group was ameliorated in the first week. Moreover, the score in combined group was less than that in control group from the 2th week. BPRS score in combined group lowered obviously. Meanwhile, BPRS score in combined group was less than that in control group. The curative ratio in combined group was 40% and higher than control group with the value of 20% （P〈0.05）. The TESS score in two groups were comparable （P〉0.05）. Conclusion Paroxetine combined risperidone were more potent and safe than risperidone on patients with acute schizophrenia patients complicated with depression.
