血清降钙素原对多发性骨折术后感染的诊断价值

目的探讨血清降钙素原（PCT）对多发性骨折术后感染早期的诊断价值。方法采集2011年3月～2012年3月沈阳市骨科医院骨科和重症监护病房（ICU）接诊的多发性骨折并发术后感染的50例患者的血清和术后未发生感染的50例患者的血清。采用全自动免疫荧光分析法检测血清PCT水平。结果感染组血清PCT水平高于非感染组,有显著性差异（P〈0.01）。血清PCT检测对细菌性感染的敏感性为96.5%,特异性为95.5%,阳性预测值为96.0%,阴性预测值为98.0%。PCT的ROC曲线下面积为0.972。结论血清PCT的检测有助于多发性骨折术后感染的早期诊断及鉴别诊断,并可作为判断预后的指标。

The diagnosis value of serum procalcitonin in multiple fractures complicated with postoperative infection

Objective To investigate the diagnosis value of serum procalcitonin （PCT） in early stage of multiple fractures complicated with postoperative infection.Methods The patient＇s serum of multiple fractures complicated with postoperative infection and without postoperative infection was delivered from orthopaedics and intensive care unit （ICU） of Shenyang Orthopaedic Hospital.The serum PCT level was detected with automatic immunofluorescence assay.Results The serum PCT level of infetion group was higher than that of non-infection group （P0.01）.The sensitivity of serum PCT to bacterial infection was 96.5%,the specificity was 95.5%,the positive predictive value was 96.0%,the negative predictive value was 98.0%.The area under ROC curve was 0.972.Conclusion The detection of serum PCT is helpful to early diagnosis and differential diagnosis of multiple fractures complicated with postoperative infection,and can be used as a prognostic indicator.
