腹内压监测在严重多发伤中的临床应用

目的 研究腹内压(IAP)检测在严重多发伤诊断、监测和治疗中的临床价值.方法 分别分析比较严重和轻度多发伤的腹内压水平、病死率、比较重度和轻度腹内压升高患者的MODS评分、器官功能障碍发生率和病死率.结果 严重多发伤患者的腹内压水平明显高于对照组,差异有显著性意义(P＜0.05),且腹内压水平与严重多发伤患者的MODS评分、器官功能障碍和病死率存在显著相关性.结论 腹内压升高与严重多发伤患者的病情及预后有密切关系,对严重多发伤患者入院后进行腹内压测定有助于多发伤患者病情的评估和预后的判断.

Role of intra-abdominal pressure in evaluating condition and prognosis of multi-trauma patients

Objective To investigate the change of intra-abdominal pressure (IAP) and its clinical significance in patients with severe multi-trauma. Methods According to injury severity score(ISS), patients with multiple injury were classified to detect the IAP; and according to IAP, 42 patients with severe multiple injury were classified to account MODS Score, incidence of organ dysfunction and prognosis.Results The levels of IAP after admission were significantly higher in the cases of severe multiple injury than those of control group (P<0.05), the levels of IAP was positively correlated with MODS score, incidence of organ dysfunction and prognosis of severe multi-trauma patients.Conclusion There is a close relationship between IAP and condition and prognosis of severe multi-trauma patients. Monitoring of IAP of severe multi-trauma patients after injury is helpful for evaluating their condition and prognosis.
