重症监护病房患者低磷血症的临床分析

目的探讨低磷血症对重症监护病房(ICU)内患者的影响。方法回顾性调查ICU内患者,血磷<0.30 mmol/L的患者进入重度低磷血症组,血磷0.30⁰.80 mmol/L的患者进入轻中度低磷血症组,血磷正常30例患者作为对照组。观察各组患者的血糖、血钾、血钠、血钙、血白细胞、白蛋白、急性生理功能和慢性健康状况评分系统Ⅱ(APACHEⅡ)评分、机械通气时间、病死率等指标。结果重度低磷血症组APACHEⅡ评分、机械通气时间、病死率均较血磷正常组明显增高(P<0.05),其余各组间各指标均无明显差异。结论重度低磷血症提示患者病情严重,预后不良。发现低磷应及时纠正,可能对改善患者预后有益。

The Clinical Analysis of Patients with Hypophosphatemia in Intensive Care Unit

Objective To investigate the impact of hypophosphatemia in critically ill patients in intensive care unit（ICU）. Methods The critically ill patients in ICU were analyzed retrospectively. The patient with serum phosphorus level〈0.30 mmol/L were grouped into the severe hypophosphatemia group, with serum phosphorus level within 0.30-0.80 mmol/L were grouped into the mild to medium hypophosphatemia group, 30 cases of critically ill patients with normal serum phosphorus as control group. Blood glucose, potassium, sodium, calcium, white blood cells count, albumin, acute physiology and chronic health evaluation score 11 （APACHE II ）, duration of mechanical ventilation, mortality were compared between the three groups. Results APACHE II score, duration of mechanical ventilation, mortality of the severe hypophosphatemia group were significantly higher than of control group（P〈0.05）. The other index between the each groups were no significantly differences. Conclusion The critically ill patients with severe hypophosphatemia prompted more serious illness, the prognosis is poor. The prompt supplying phosphorus is possible useful to improve the prognosis of patients.
