糖尿病合并肺结核的临床及影像学诊断

目的:回顾性分析糖尿病合并肺结核的临床表现、X线和CT表现特点。方法:50例糖尿病肺结核患者均摄DR胸部正侧位片,其中35例行CT检查。结果:50例糖尿病肺结核患者起病急骤、进展快、消瘦乏力明显,发热与肺部病变程度不一致,均有不同程度咳嗽、咯痰和咯血。其X线表现为:大片状及多发小片融合性高密度阴影(45例),中心密度浓,边缘模糊浅淡,空洞(36例),合并胸腔积液(14例),粟粒状弥漫结节影(2例)。病灶多肺段分布(38例),可发生于一般结核少见部位。CT表现基本同X线。部分病例经治疗未见明显好转或复发。结论:糖尿病并发肺结核的临床表现与单纯肺结核、单纯糖尿病的临床表现有不同,大多数合并糖尿病的肺结核患者起病急骤、进展快、消瘦乏力明显,发热与肺部病变程度不一致。糖尿病合并肺结核的影像表现以大片状及多发小片融合性高密度影,多发空洞,多肺段病变,少见部位高发病率为特点。
