急性阑尾炎的超声声像图特征及扫查技巧分析

摘    要:目的:探讨急性阑尾炎的超声声像图特征和检查技巧。方法:回顾分析术前采用高频、低频、经阴道探头相结合并联合运用加压、最痛点扫查等方法组合扫查,术后经手术病理证实的159例急性阑尾炎患者超声诊断资料。结果:组合扫查诊断急性阑尾炎符合率93.7%。急性阑尾炎声像图直接征象:阑尾肿大和阑尾周尾脓肿;间接征象:回盲处肠管异常改变,右下腹淋巴结肿大,腹腔积液,粪石,网膜高回声团,超声麦氏征,右下腹腹膜线增厚。结论:超声医生快捷、准确诊断急性阑尾炎,除了要熟练掌握其声像图直接、间接征象外,组合扫查是关键。

Abstract:Objective To investigate the ultrasonic features and scan skills in the acute appendicitis.Method 159 patients′ ultrasound data were analyzed,who were diagnosed as acute appendicitis by pathology after surgery.Some techniques of ultrasound were used on the diagnoses,such as high frequency,low frequency,vaginal probe,pressure scanning,the pain point scanning and so on.Results There was a high according rate of diagnosis with combination scanning on acute appendicitis,and the according rate was about 93.7%.The direct characteristics of ultrasound imaging on acute appendicitis were appendiceal enlargement and periappendicular abscess,and the indirect characteristics were the abnormal changes of ileocecal intestinal,the enlargement of lymphnodes in lower right abdomen,seroperitoneum,fecalith,high echo of omentum,ultrasound-Maxwell syndrome,the abdomino-membrane line of lower right abdomen becoming thick.Conclusion It′s important for us to know the direct and indirect characteristics of ultrasound imaging on acute appendicitis,and to use the combination scanning techniques on acute appendicitis.
