心肌功能综合指数评价慢性肺心病患者右室功能的临床研究

目的探讨心肌功能综合指数(MPI)评价慢性肺心病患者右心功能的临床价值.方法慢性肺心病患者40例,正常对照组50例,应用多普勒超声心动图检查,记录三尖瓣舒张期和右心室流出道收缩期脉冲多普勒血流频谱,测量MPI.结果①慢性肺心病患者等容舒张期时间及等容收缩期时间明显延长、射血时间明显缩短,导致MPI明显升高(0.94±0.33 vs 0.30±0.06,P<0.0001);②慢性肺心病组MPI与年龄、肺动脉收缩压呈正相关.结论 MPI是评价慢性肺心病患者右心功能的简便而准确的多普勒超声新指标.

Usefulness of the myocardial performance index for assessing right ventricular function in chronic cor pulmonale patients

Objective To investigate the clinical value of myocardial performance index (MPI) in assessment of right ventricular function in patients with chronic cor pulmonale. Methods Forty patients with chronic cor pulmonale and 50 normal subjects underwent echocardiographic examination. Defined as the sum of isovolumic relaxation time and isovolumic contraction time derived by ejection time, MPI was measured from tricupid inflow and right ventricular outflow Doppler velocity profiles recorded during routine echocardiography. Results The isovolumic relaxation and isovolumic contraction times were prolonged and ejection time was shortened in the patients with chronic cor pulmonale compared with those in normal subjects, resulting in a marked increase of MPI (0.94±0.33 vs 0.30±0.06, P<0.001). MPI had no correlation with heart rate and blood pressure in normal subjects and the patients with chronic cor pulmonale, but correlated positively with age in the patients with chronic cor pulmonale. MPI also correlated positively with the pulmonary artery systolic pressure. Conclusion MPI is a conceptually new, simple and reproducible Doppler index in patients with chronic cor pulmonale.
