一期前后路手术治疗上胸椎肿瘤

目的探讨Ⅰ期前后路手术治疗上胸椎肿瘤的价值。方法对4例上胸椎肿瘤患者行前后路Ⅰ期手术，前路肿瘤切除椎管减压后行椎问植骨并钢板内固定。后路肿瘤切除后以钩棒或钉棒系统固定。结果4例患者均安全度过围手术期，随访6-22个月，术后脊髓功能均有不同程度改善。结论对于三柱同时破坏的上胸椎肿瘤患者，Ⅰ期前后路手术有利于切除肿瘤、椎管减压和恢复脊柱的稳定性。

Surgical treatment of upper thoracic spine tumor by one stage anterior and posterior approach

Objective] To evaluate the clinical resuks of one stage anterior and posterior approach in the treatment of upper thoracic spine tumor. Methods] 4 cases of upper thoracic spine tumor underwent resection, spinal cord decompression, restoration of the stabilization of vertebrae through one stage anterior and posterior approach, their clinical and radiographic results were reviewed retrospectively. Results] All patients stood surgery well, the follow-up period ranged from months, all patients had their neurological improvement at different level, according to Frankel grading system, one patient improved from grade A pre-operation to C post-operation, of 2 cases with grade B, one patient improved to D and the other to E, one case kept the same grade before and after operation. Conclusion] One stage anterior and posterior approach is essentially safe and effective for adequate tumor removal, neural decompression and stabilization of the spinal column.
