血氧饱和度对早产儿视网膜病发病率的影响

摘    要:目的:分析不同血氧饱和度对早产儿视网膜病(Rop)发病率的影响。方法:选择2010年1月到2012年12月我院NICU收治的650例早产儿作为研究对象,根据平均血氧饱和度水平将早产几分为低血氧饱和度组409例(SaO,91%~95%)和高血氧饱和度组241例(SaO:96%一100%),观察住院期间血氧饱和度和视网膜病发病率的情况。结果:高血氧饱和度组早产儿ROP发病率18.1%,高于低血氧饱和度组的发病率的4.6%(P〈0.01),并且高血氧饱和度组早产儿ROP各期的患病例数都明显高于低血氧饱和度组早产儿(P〈0.05)。Logistic多元回归分析结果显示,高血氧饱和度早产儿ROP发病相对危险度值最高(OR=2.674,95%CI:1.246,6.554)。结论:早产儿吸氧时血氧饱和度应尽量控制在91%一95%之间,否则可增加ROP的发生。

Abstract:Objective: To analyze the effect of different arterial oxygen saturation on incidence of retinopathy(ROP) in premature infants. Methods. Sixty hundreds and fifty cases of premature infants were selected as research subjects from January 2010 to December 2012 in NICU, and the data about ROP and arterial oxygen saturation (SaO2)were also collected. According to the average level of SaO2, these premature infants were divided into a low SaO2 group ( SaO2 91% - 95%, n = 409 ) and a high SaO2 group ( SaO2 96% ~ 100%, n=241 ). The arterial oxygen saturation and the incidence of retinopathy was observed. Results: The incidence of ROP of children in high SaO2 group was significant higher than those in low SaO2 group ( 18.1% vs 4.6%, P〈0.01 ). The similar result was found in the number of each stage of ROP between the two groups ( P〈0.05 ). Logistic regression analysis also showed that the high SaO2 was the highest relative risk ( OR = 2. 674, 95% CI: 1. 246, 6. 554 ) for the incidence of ROP. Conclusions: The Sa02 in preterm children should be controlled between 91% ~ 95% to decrease the incidence of ROP.
