不同分娩方式对产后出血的影响

目的分析产后出血的临床特点及自然分娩和剖宫产的出血特点,并探讨有效的治疗方法。方法将625例产妇根椐分娩方式的不同分为自然阴道分娩组360例和剖宫产组265例。比较2组产后出血率、休克率、出血量及出血时间。结果自然阴道分娩组产后出血率及休克率均低于剖宫产组,出血量少于剖宫产组,差异均有统计学意义(P<0.01)。剖宫产组3～24h出血率低于自然阴道分娩组,晚期出血率高于自然阴道分娩组,差异均有统计学意义(P<0.05)。结论剖宫产较自然阴道分娩更易发生产后出血,且出血量较多。因此,临床行剖宫产时为减少产后出血必须严格手术操作。

Effect of different mode of delivery on postpartum hemorrhage

Objective To analysis the clinical characteristic of postpartum hemorrhage and bleeding characteristic of natural labor and cesarean section, and to explore effective treatment. Methods By the mode of delivery, 625 cases maternal were divided into natural vaginal delivery group （360 cases） and cesarean section group （265 cases）. Compared the rate of postpartum hemorrhage, shock rate, amount of bleeding and bleeding time of two groups. Results The rate of postpartum hem- orrhage and shock of natural vaginal delivery group were lower than those of cesarean section group, amount of bleeding was less than that of cesarean section group, the difference were statistically significant（P 〈 0.01 ）. 24h bleeding rate of caesarean section group was lower than that of natural vaginal delivery group,late bleeding rate was higher than taht of natural vaginal de- hvery group,the difference were statistically significant（ P 〈 0.05）. Conclusion Cesarean section comparing with natural va- ginal delivery is more likely to happen the bleeding after delivery, the bleeding more. Therefore, in clinical on when to reduce postpartum hemorrhage of cesarean delivery must strictly operation.
