妊娠期糖尿病孕早期微量元素检测对预后的指导作用

摘    要:目的探讨妊娠期糖尿病孕早期微量元素检测对预后的指导作用。方法选取2013年1月—2014年12月在海军总医院产科门诊就诊的280例孕早期孕妇,包括妊娠期糖尿病患者80例(观察组)和正常妊娠无并发症孕妇200例(对照组),都进行血清微量元素(铜、锌、钙、镁、铁、铅)检测与预后的观察。结果观察组的血清铜、锌、钙、镁、铁含量明显低于对照组(P<0.05),2组血清铅均无检出。观察组的血清铜、锌、钙、镁、铁含量下降发生率明显高于对照组,差异有统计学意义(P<0.05)。观察组的巨大儿、产后出血与新生儿窒息发生率都明显高于对照组(P<0.05)。直线相关分析显示,巨大儿、产后出血与新生儿窒息发生情况与血清铜、锌、钙、镁、铁含量存在明显相关性(P<0.05)。结论妊娠期糖尿病在孕早期表现为血清微量元素含量下降,发生率都比较高,同时巨大儿、产后出血与新生儿窒息等预后不良发生率增加。妊娠期糖尿病孕早期微量元素的检测对预后有一定的指导作用。

Abstract:ObjectiveTo investigate the guidance values of trace elements for the prognosis of gestational diabetes mellitus at early pregnancy. MethodsFrom January 2013 to December 2014, 280 cases of pregnant women included 80 gestational diabetes mellitus cases (observation group) and 200 normal pregnancy women without complications (control group), were carried out trace elements (copper, zinc, calcium, magnesium, iron, lead) detection and prognosis of observation. ResultsThe levels of serum copper, zinc, calcium, magnesium, and iron in the observation group were significantly lower than those in the control group (P<0.05), the two groups had no detectable serum lead levels. The serum copper, zinc, calcium, magnesium, iron, in the treatment group were all significantly higher those in control group with statistically significant (P<0.05). The rates of huge children, postpartum hemorrhage and newborn asphyxia in the treatment group were significantly higher than those in the control group (P<0.05). In the observation group, linear corre lation analysis showed that the huge children, postpartum hemorrhage and newborn asphyxia were correlated to the serum copper, zinc, calcium, magnesium, and iron levels (P<0.05). ConclusionGestational diabetes mellitus is showed decreased serum trace elements, the lack rates are higher, and the rates of huge children, increased postpartum hemorrhage and newborn asphyxia are increased. The detection of serum trace elements for gestation diabetes mellitus at early pregnancy has guidance for prognosis.
