儿童急性下呼吸道感染非典型病原体病原学分析

目的 调查分析儿童急性下呼吸道感染非典型病原体的流行情况,为临床合理用药提供病原学依据。方法采集1 759例急性下呼吸道感染患儿的血清标本,采用间接免疫荧光法进行呼吸道九联检的检测,得出病原体分布情况,并对其结果进行分析。结果在1 759例儿童急性下呼吸道感染患者中,总阳性率为38.7%,其中检出率最高的是肺炎支原体,占19.6%,其次为嗜肺军团菌12.3%,呼吸道合胞病毒4.1%,未检出Q热立克次体和肺炎衣原体。共发现混合感染患者47例,混合感染率达6.9%。结论本地区儿童急性下呼吸道感染的非典型病原体以肺炎支原体和嗜肺军团菌较常见,亦存在其他病原体感染,不同地区的病原菌分布存在着一定的差异,在临床用药中应根据病原体的不同选择用药。

Analysis of Atypical Pathogens in Children with AcuteLower Respiratory Tract Infections

Objective To analyse the distribution of atypical pathogens in children with acute lower respiratory tract infections and give guidance of therapy based on our study.MethodNine tests of respiratory tract of 1 759 patients which serum was collected were examined by indirectimmuno fluorescence assay.The distribution of pathogens was analysed.ResultsIn 1 759 patients,the total positive rate of pathogens was 38.7%,and Mycoplasma pneumonia was the most common pathogen of infection(19.6%),followed by Legionella pneumophila(12.3%),Respiratory syncytial virus(4.1%).We didn''t detect Richettsia query and Chlamydia pneumoniae.Mixed infections were found in 47 cases,with the positive rate of 6.9%.ConclusionThe common atypical pathogens of acute lower respiratory tract infections are Mycoplasma pneumonia and Legionella pneumophila.Species difference exits in different ages.Treatments should be given based on pathogens.
