老年壶腹周围癌外科治疗体会(附91例报告)

本文总结老年壶腹周围癌（ＶＰＣ）手术治疗体会。９１例老年ＶＰＣ全部接受手术治疗，其中根治术４４例、姑息手术３５例、剖腹探查术１２例。手术切除率：根治术４８．４％、姑息３８．４％、剖腔探查术１３．２％。围手术期死亡率８．８％、并发症发生率２９．７％。根治术存活〉５年１０例，姑息术３年内全部死亡，冲腹探查术生存未超过１年。充分术前准备，老年ＶＰＣ可手术治疗。围手术期死亡与病员自身状态、并存疾病密切相关

Therapeutic Experiences of Periampullar Carcinoma in Senile Patients

The results of operative therapy of periampullar carcinoma(VPC) in senile patients were reported.91 patients were treated by surgical operations,44 cases(48 4%) of them underwent radical operations,35 cases (38 4%),palliative operation,12 cases (13 2%), laparotomy exploration.The results showed that the mortality was 8 8% in perioperation.The complications were 29 7%.In 10 cases of radical operation,survival time was longer than 5 years.The cases of palliative operation died within 3 vears.Survival time of all cases of exploration shorter than one year.It suggests that the operative therapy of VPC for senile patients needs sufficient preparation before operation.The mortality of perioperation is associated with self-tolerance and concomitant diseases.The radical operations are more effective than palliative ones.
