大龄语前聋人工耳蜗植入者听觉语言康复效果分析

目的报道大龄语前聋患者人工耳蜗植入术及术后听觉语言康复的效果。方法经术前听力学、影像学、心理及智力评估,筛选33例年龄7~33岁的重度-极重度语前聋患者采用面隐窝进路的常规术式行单侧人工耳蜗植入,术后接受语言训练,随访4年,采用开放式和闭式双音节词表测试患者的言语识别率。结果 33例患者术后1年声场测听语言频率啭声听阈平均(28.62±2.37)dBHL。借助唇读,术后1年闭式词表识别率平均达到70.03%,术后4年闭式词表识别率平均达到82.12%。不借助唇读术后1年闭式词表识别率平均达到61.18%,术后4年达到72.67%。术后4年开放式词表识别率借助唇读平均达到77.94%;不借助唇读开放式词表识别率达到67.85%。结论人工耳蜗植入能提高大龄语前聋患者生活质量,多数病例仍能得到较好的言语分辨率,对信息交流仍有很大帮助。本组患者和家长权衡术后的获益和手术费用投入,90.90%的家庭认为值得进行人工耳蜗植入。

Cochlear implantation and rehabilitation in prelingually deafened adolescents and young adults
